Sexueller Missbrauch – ein Thema für Proktologen et al.

Zusammenfassung

Der sexuelle Missbrauch von Kindern und Jugendlichen hat eine ungeahnt hohe Inzidenz und oft negative Auswirkungen auf die Lebensqualität im Erwachsenenalter, insbesondere bei Frauen, die am häufigsten betroffen sind. Einige der Folgen, vornehmlich im Gastrointestinaltrakt, sind die Syndrome von Reizdarm und obstruktiver Defäkation mit paradoxer Kontraktion des M. puborectalis (Beckenbodendyssynergie). Ein weiteres Symptom ist der scheinbar unerklärliche Schmerz im kleinen Becken („pelvic/anal pain syndrome“) oder verschiedene Körperregionen betreffend (Fibromyalgie). Natürlich steht der psychische Hintergrund einer wirksamen Therapie entgegen, solange der in der Vergangenheit liegende, verdrängte Missbrauch der Opfer nicht entdeckt ist. Dies erklärt insbesondere die Fehlschläge der Chirurgie, von peranalen Exzisionen mit oder ohne Klammerapparat bis zur Rektopexie, die allesamt das Potenzial der Frustration und das Risiko von Komplikationen bergen. Daher muss von Allgemeinmedizinern und Fachärzten nach dem Missbrauch gefragt werden, wenn – neben persistierenden gastrointestinalen Beschwerden ohne organische Ursache – eine oder mehrere der folgenden Charakteristika zutreffen: psychische Auffälligkeit, eine Vorgeschichte frustraner Therapieversuche, häufiger Arztwechsel („doctor shopping“), ungenügende Compliance der Betroffenen. Für die Anamnese sind in solchen Fällen zunächst keine Kenntnisse der Psychiatrie erforderlich. Es genügt, die Tatsache des Missbrauchs festzustellen, um Patienten dann Psychologen, Fachärzten für Psychiatrie bzw. Psychosomatik und Physiotherapeuten zuzuweisen.
