Zusammenfassung

Die zunehmende Problematik der Adipositas hat zu einer starken Zunahme der chirurgischen Adipositaschirurgie (bariatrische Chirurgie) geführt. Als Alternative und als Zusatz zur Chirurgie wurden neue endoskopische Eingriffe entwickelt. Diese endoskopischen bariatrischen und metabolischen Therapieverfahren werden in der Behandlung der Adipositas an Bedeutung gewinnen. Zur Verfügung stehen bereits jetzt der Magenballon, die Aspirationstherapie, die endoskopische Magenverkleinerung als Alternative zum chirurgischen Magenschlauch und das mukosale Resurfacing. Letzteres ist eine einmalige endoskopische ablative Behandlung des Duodenums, die eine wichtige Rolle in der Behandlung des Typ-II-Diabetes einnehmen könnte. Implantate wie der Magenballon und die Aspirationstherapie haben nur einen vorübergehenden Effekt und sind in der Schweiz wenig etabliert. Der endoluminale Bypassschlauch war ein sehr effektives Verfahren, musste jedoch wegen Komplikationen wieder vom Markt genommen werden. Interessant könnte in der Zukunft der Einsatz endoskopischer Anastomosen werden. Neben diesen endoskopischen bariatrischen Eingriffen spielt die endoskopische Verkleinerung einer erweiterten gastrojejunalen Anastomose nach chirurgischem Magenbypass bei sekundärer Gewichtszunahme eine Rolle.
