Dynamische Stabilisierung der Wirbelsäule

Zusammenfassung

Die intervertebrale Fusion ist seit Jahrzehnten der Goldstandard in der Behandlung unterschiedlicher Erkrankungen der Wirbelsäule. Obwohl die Fusionsraten durch Verwendung von Pedikelschrauben und interkorporellen Cages deutlich gesteigert werden konnten, treten nach Wirbelsäulenfusionen häufig Anschlusssyndrome in der Langzeitbeobachtung auf. Dem soll durch dynamische Implantate entgegengewirkt werden. Sie sollen eine Restbeweglichkeit erhalten und so eine Entlastung des Nachbarsegments bewirken. Mittlerweilte steht dem Wirbelsäulenchirurgen eine umfassende Palette an dynamischen Systemen mit unterschiedlicher Wirkung zur Verfügung. Zahlreiche Studien bestätigen sowohl eine positive als auch eine negative Wirkung dieser Systeme, sodass es für den Anwender schwierig ist, daraus einen entsprechenden Behandlungsalgorithmus abzuleiten. Dieser Beitrag gibt einen Überblick über die einzelnen Ansätze und Systeme.
