Zusammenfassung

Die Kawasaki-Erkrankung (KE) wird der Gruppe der Vaskulitiden zugeordnet. Die Ursache ist unklar. Die Diagnose wird klinisch gestellt. Gefordert wird unklares Fieber über 5 Tage und 4 von 5 der klinischen Symptome: 1. bilaterale nicht eitrige Konjunktivitis; 2. zervikale Lymphadenopathie; 3. polymorphes Exanthem; 4. Mundschleimhautveränderungen, Lacklippen, Erdbeerzunge; 5. Auffälligkeiten an Händen und Füßen (Erythem, Ödem, später Schuppungen). Verläufe mit weniger als 4 Kriterien werden als inkomplette KE bezeichnet. Die Standardtherapie besteht aus hoch dosierter intravenöser Immunglobulingabe (2 g/kgKG als Einzelgabe) und Acetylsalicylsäure. Bei schweren Verläufen und Risikofaktoren sind Glukokortikoide in die primären therapeutischen Überlegungen einzubeziehen. Von großer Bedeutung ist die Beteiligung der Koronararterien, was zu Langzeitproblemen führen kann.
