Die neue internationale PCOS-Leitlinie – welche Neuerungen bringt das erste Update?

Zusammenfassung

Das Syndrom polyzystischer Ovarien (PCOS) gehört zu den häufigsten Endokrinopathien der Frau und ist eine chronische Erkrankung mit Langzeitfolgen für Reproduktion, Metabolismus und kardiovaskuläre Gesundheit. Im ersten Update zur internationalen Leitlinie zum PCOS von 2018 wurde neu als Diagnosekriterium das Anti-Müller Hormon (AMH, nicht bei Jugendlichen!) aufgenommen, es kann das sonomorphologische Bild polyzystischer Ovarien ersetzen. Ein besonderer Fokus im Update sind die kardiovaskulären und metabolischen Langzeitfolgen. Ebenfalls neu ist eine Aufwertung von Inositol und die Aufnahme von pharmakologischen Adipositastherapeutika in die Therapieempfehlungen. Zur Zyklusregulierung und antiandrogenen Therapie werden hormonelle Kontrazeptiva in Abwägung der Risiken empfohlen. Letrozol wurde in seiner Position als medikamentöse First-line-Therapie zur Ovulationsinduktion bestärkt. Trotz der hohen Effektivität und Sicherheit kann Letrozol jedoch weiterhin nur off-label verordnet werden.
