Zusammenfassung

Hypertensive Erkrankungen (HES) gehören zu den häufigsten Komplikationen in der Schwangerschaft. Auch in den industrialisierten Ländern bleibt die Präeklampsie eine der wichtigsten Ursachen für die maternale Mortalität.

Die Definition der Präeklampsie wurde hinsichtlich ihrer Pathophysiologie von vielen Gesellschaften verfeinert. Es gilt, zwischen früher und später Manifestation sowie plazentarer und maternaler Genese zu unterscheiden.

Der Fortschritt in der Forschung bezüglich angiogener Faktoren hat in einigen Leitlinien zu adaptierten Empfehlungen zum Screening im 1. Trimenon sowie der Diagnose und Kurzzeitprädiktion der Präeklampsie geführt. Die Kombination von Angiogenesefaktoren zusammen mit anamnestisch bestehenden Risikofaktoren, der Dopplersonografie der Aa. uterinae und dem mittleren arteriellen Blutdruck hat die Risikostratifizierung wesentlich verbessert (siehe Abschn. 6).

Nach wie vor ist die einzige kausale Therapie der Präeklampsie die Entbindung. Modernes Management berücksichtigt von der Früherkennung mit der Erfassung von Risikofaktoren über ein individualisiertes Betreuungskonzept in der Schwangerschaft und einer zeitgerechten Entbindung auch die postnatale Versorgung, insbesondere die Beratung bezüglich Langzeitkomplikationen für Mutter und Kind.
