Zusammenfassung
Die fetale und neonatale Alloimmunthrombozytopenie (FNAIT) wird, ähnlich der fetalen und neonatalen Alloimmunanämie, durch plazentagängige maternale IgG-Alloantikörper gegen Antigene auf der Zelloberfläche der fetalen Thrombozyten hervorgerufen. Bei schweren Verläufen dieser Erkrankung kann es bereits antenatal oder perinatal nicht nur zu einer schweren Thrombozytopenie, sondern auch zu einer Hirnblutung kommen, die zum perinatalen Tod und bei Überleben häufig zu schweren bleibenden neurologischen Defiziten führt. Im Gegensatz zur Blutgruppeninkompatibilität kann es bereits während der ersten Schwangerschaft nach Sensibilisierung zu einer schweren FNAIT kommen. Sofern das Risiko einer FNAIT besteht und bekannt ist, kann eine Sekundärprävention mit wöchentlichen intravenösen Infusionen hoch dosierter Immunglobuline (IVIg) erfolgen, die eine schwere FNAIT, insbesondere aber eine Hirnblutung verhindert.
