Patients with serrated polyposis syndrome (SPS) have an increased risk of colorectal cancer (CRC) and advanced neoplasia. In a multicenter cohort of 152 SPS patients who had undergone a prior clearing colonoscopy, the cumulative incidence of CRC and advanced neoplasia at 3 years was 3.1 % and 42.0 %, respectively, with the presence of an advanced serrated lesion at baseline and fulfillment of both criteria I and III being independent risk factors.
