Zusammenfassung

Adipositas ist eine multifaktoriell bedingte Erkrankung, bei deren Pathogenese genetische Faktoren, Überernährung, Bewegungsmangel sowie vielfältige andere Mechanismen eine Rolle spielen. Etliche Studien des vergangenen Jahres leisteten nicht nur einen Beitrag zum besseren Verständnis der Entstehung von Adipositas, sondern auch zur Assoziation mit Begleiterkrankungen, insbesondere kardiovaskulären Komplikationen. Entsprechend den aktuellen Leitlinien sind Ernährungs-, Bewegungs- und Verhaltenstherapie Grundlage einer Behandlung von Adipositas, wobei die hier bevorzugt zum Einsatz kommenden Strategien immer wieder Gegenstand der Diskussion sind. Während der Stellenwert einer Pharmakotherapie von Adipositas zuletzt eher rückläufig erscheint, gilt für die Adipositaschirurgie das Gegenteil. Auch die publizierte Datenlage zur chirurgischen Therapie war in der jüngeren Vergangenheit eher günstig.

Abstract

Obesity is a multifactorial disease, with its pathogenesis including genetic factors, over-eating, physical inactivity and a variety of other factors. Numerous studies published recently contributed to a better understanding not only of the mechanisms leading to obesity, but also of the association between obesity and accompanying diseases, in particular cardiovascular complications. According to current guidelines, dietary, physical and behavioral therapy form the basis for the treatment of obesity. There is, however, an ongoing debate about the different strategies that are currently used in the clinical setting. While the relevance of pharmacotherapy for obesity appears to have declined, the opposite can be observed for bariatric surgery. Recently published studies with favorable data support the use of surgical strategies in obese subjects when indicated.
