Zusammenfassung
Die akute Lungenarterienembolie (LAE) ist eine häufige Kreislauferkrankung. Die zeitnahe Erkennung und Therapie der akuten LAE sind prognostisch wichtig. Diagnostik und Akuttherapie erfolgen nach standardisierten Vorgaben. Bei klinischem Verdacht und niedriger Wahrscheinlichkeit sollen D‑Dimer-Spiegel bestimmt werden und bei erhöhten Spiegeln sollte eine thorakale Angio-Computertomographie (CT) erfolgen. Bei hoher klinischer Wahrscheinlichkeit sollte die CT-Darstellung der Pulmonalgefäße ohne vorherige Messung der D‑Dimere durchgeführt werden. Schon bei klinischem Verdacht auf eine LAE sollte eine Antikoagulation initiiert werden. Nur bei einer Hochrisiko-LAE wird die systemische Thrombolyse empfohlen. Die Dauer einer Antikoagulation richtet sich danach, ob es sich um ein provoziertes oder unprovoziertes Ereignis und um ein Erst- oder Rezidivereignis handelt. Die Frage nach einem Screening auf okkulte Tumoren und andere Erkrankungen sowie die Beurteilung einer persistierenden Belastungsintoleranz sind im Alltag wichtige, aber zum Teil nicht gänzlich geklärte Fragen. Die Erkennung „akut-auf-chronischer“ Embolien ist prognostisch wichtig. Eine Nachsorge nach einer LAE erscheint sinnvoll, ist aber bisher noch nicht standardisiert und in der Breite implementiert. Aufgaben der Nachsorge sind die Festlegung der Antikoagulationsdauer nach Einschätzung der Rezidivwahrscheinlichkeit, die Erfassung okkulter Tumoren und anderer Komorbiditäten sowie die Einordnung einer häufig persistierenden Belastungsintoleranz. Bei persistierender Belastungsintoleranz nach LAE ist immer eine CTEPH („chronic thromboembolic pulmonary hypertension“) auszuschließen. Ausschluss-Differenzialdiagnosen sind Trainingsmangel, Komorbiditäten und eine posttraumatische Belastungsstörung.
Abstract
Acute pulmonary embolism (PE) is frequent. Prompt detection and treatment have an important prognostic impact. If a pulmonary embolism is suspected, prompt anticoagulation is recommended. Diagnosis and treatment of acute PE should be performed according to the current guidelines. If acute PE is suspected but clinical probability seems to be low, the measurement of D‑dimer levels is recommended. In case of elevated D‑dimer levels or if clinical probability is high, a thoracic computed tomography should be performed. Systemic thrombolytic therapy is only recommended for patients with acute high-risk PE. To determine the duration of anticoagulation, it is important to know whether the PE was provoked or unprovoked and whether the PE is the first or a recurrent event. Detection of acute on chronic episodes of PE has a relevant prognostic impact. Screening for occult malignancies and other comorbidities as well as management of persistent symptoms and standardized follow-up after acute PE are important issues. However, standard recommendations are lacking. In patients with persisting exercise limitations following an acute PE, chronic thromboembolic pulmonary hypertension (CTEPH) should be excluded. Diagnosis of muscular deconditioning, other cardiopulmonary comorbidities, and posttraumatic stress disorder can only be made after having excluded CTEPH.
