Zusammenfassung
Hintergrund
Die intravesikale Instillation von Bacillus Calmette-Guérin (BCG) ist eine anerkannte Strategie zur Reduktion des Rezidiv- und möglicherweise auch des Progressionsrisikos des „high risk“ nicht-muskelinvasiven Blasenkarzinoms (NMIBC). Es treten jedoch belastende Nebenwirkungen auf. Außerdem können die Tumoren BCG-refraktär oder -unempfindlich sein. Diese Tumoren haben ein besonders hohes Rezidiv- und Progressionsrisiko, so dass zwischen Zystektomie und konservativen Therapiealternativen abgewogen werden muss.
Fragestellung
Wir beschreiben hier die aktuellen Empfehlungen für die Behandlung eines NMIBC mit BCG sowie Alternativen bei BCG-Versagen.
Material und Methode
Es wird eine Literaturrecherche zu aktuellen Behandlungsoptionen und deren Alternativen mit Hilfe von Primärliteratur und Leitlinienempfehlungen durchgeführt.
Ergebnisse und Schlussfolgerung
Bei High-risk-NMIBC bleibt die Instillationstherapie mit BCG „standard of care“. Die Anwendung erfolgt nach einem Standardschema in Bezug auf Dosis und Dosierungsintervalle (Induktion: wöchentliche Instillation für 6 Wochen, Erhaltung: wöchentliche Instillation für 3 Wochen, 3, 6 und 12 Monate nach Beginn der BCG-Therapie, sowie zusätzlich, bei High-risk-NMIBC 18, 24, 30 und 36 Monate nach Beginn der BCG-Therapie). Künftige Behandlungsoptionen bei BCG-Versagen sind systemisches (i.v.) Pembrolizumab (in den USA bereits zugelassen) und möglicherweise eine intravesikale Gentherapie mit Nadofaragen Firadenovec. Aktuelle randomisierte Studien prüfen außerdem den Stellenwert von PD-(L)1(„programmed cell death [ligand] 1“)-Immun-Checkpoint-Inhibitoren in Kombination mit BCG.
Abstract
Background
Intravesical instillation of bacillus Calmette-Guérin (BCG) is an accepted strategy to reduce the risk of recurrence and possibly progression of high-risk non-muscle invasive bladder cancer (NMIBC). However, side effects are not uncommon. In addition, the tumors may be BCG refractory or unresponsive. These tumors have a very high risk of recurrence and progression, so cystectomy must be weighed against conservative treatment options.
Objectives
We describe the current recommendations regarding treatment of NMIBC with BCG and alternatives for BCG failure.
Methods
Literature search on current treatment options and their alternatives with the help of mainly primary literature and guideline recommendations.
Results and conclusion
For high-risk NMIBC, instillation therapy with BCG remains standard-of-care, applied according to a standard regimen in terms of dose and dosing intervals (induction: weekly instillation for 6 weeks, maintenance: weekly instillation for 3 weeks, 3, 6 and 12 months after initiation of BCG therapy plus, for high-risk NMIBC, 18, 24, 30 and 36 months after initiation of BCG therapy). Potential future treatment options for BCG failure are systemic (i.v.) pembrolizumab (FDA approved) and, possibly, intravesical nadofaragene firadenovec. Ongoing randomized clinical trials are furthermore evaluating the role of PD-(L)1 immune checkpoint inhibitors in combination with BCG.
