Zusammenfassung
Hintergrund
Die erfolgreiche Therapie der Adipositas bei Kindern und Jugendlichen ist nicht nur von großer individueller Bedeutung (Verbesserung von Gesundheit und Lebensqualität, bessere Entwicklungschancen, Vermeiden von Stigmatisierung), sondern hat langfristig auch erhebliche (gesundheits-)ökonomische Auswirkungen.
Fragestellung
Betroffen sind mehrheitlich Kinder ab dem Schulalter; eine schwierige soziale Lage und ein Migrationshintergrund erhöhen das Risiko. Der Anteil der genetischen Disposition an der Gewichtsentwicklung bei Kindern ist größer als bei Erwachsenen. Die Fragen „Wer ist schuld?“ und „Was muss verändert werden?“ erfordern deshalb eine systematische Problemanalyse und differenzielle Therapieplanung. Auf der Grundlage der aktuellen Evidenz werden realistische Therapieziele diskutiert.
Ergebnisse
Die erfolgreiche Therapie der Adipositas bei Kindern und Jugendlichen ist – abgesehen von wenigen Ausnahmen (syndromale Adipositas, extreme Adipositas bei Jugendlichen) – im Rahmen von multiprofessionellen Lifestyle-Interventionen möglich. Diese streben eine Verbesserung des Ernährungs- und Bewegungsverhaltens sowie des Selbstbewusstseins und der Stressbewältigung an. Die aktive Beteiligung der Familie ist unbedingt notwendig. Eine geringe nachhaltige Wirkung kann in aktuellen Übersichten gezeigt werden, allerdings werden Risikofamilien seltener erreicht.
Schlussfolgerung
Durch eine differenzielle Therapieplanung unter Berücksichtigung der Ressourcen der Familien soll der Behandlungserfolg verbessert werden. Dazu kann eine Modularisierung der Schulungsprogramme beitragen. Die Therapieziele müssen über die alleinige Gewichtsreduktion hinaus die Reduzierung von Komorbidität und die Verbesserung von gesundheitsbezogener Lebensqualität berücksichtigen. Gesundheitsfördernde Umgebung (Setting-Ansatz) und Vermeidung von Stigmatisierung für Betroffene als mittelbare Ziele würden ein sinnvolles Gesamtkonzept abrunden.
Abstract
Background
Successful therapy of pediatric obesity is not only of individual importance (improving health outcome and quality of life, improved chances for development, avoidance of stigmatization) but also has significant (health-related) economic effects in the long-term.
Objective
In the majority of cases children at school age or older are affected; however, difficult social conditions as well as a migration background increase the risk of obesity. In contrast to adult obesity, the influence of genetic determinants is more important in children and adolescents. Therefore, the questions “who is guilty?” and “what must be changed?” necessitate a systematic analysis of the problem and treatment planning. Realistic therapy goals for the treatment of pediatric obesity are discussed based on the current evidence.
Results
Successful therapy of pediatric obesity is, apart from a few exceptions (monogenic or syndromal obesity, extreme obesity in adolescence) possible using the setting of multiprofessional life style interventions. The aims are to improve nutritional behavior and activity as well as self-confidence and stress management. The active involvement of the patient’s family is essential. A slight durable effect could be shown in current review articles but families at risk are rarely reached.
Conclusion
By differential therapy planning, taking the resources of the family into account, the success of treatment can be improved. To reach this goal, modularization of educational programs can make a contribution. Therapy goals need to exceed the sole aim of weight reduction and should also address the reduction of comorbidities as well as improvement of the health-related quality of life. A health-promoting setting and the avoidance of stigmatization of the affected patients as immediate goals would complete a reasonable overall concept for pediatric obesity.
