Zusammenfassung
Der frühzeitige Beginn von Reanimationsmaßnahmen hat enorme Bedeutung für das Outcome eines Kindes nach einem Herz-Kreislauf-Stillstand. Daher nehmen die Basismaßnahmen sowohl durch Laienhelfer als auch durch medizinisches Fachpersonal eine herausragende Stellung in der Versorgung der Patienten ein. Reanimationsmaßnahmen, die durch Beobachter begonnen wurden, sind mit einem besseren Überleben sowie besserem neurologischem Outcome bei Erwachsenen und bei Kindern assoziiert, als Maßnahmen, die erst durch das Notfallteam initiiert wurden. Daher sind das rechtzeitige Erkennen eines kritisch kranken Patienten, der frühzeitige Beginn sowie die effektive Durchführung von Basismaßnahmen, wie Sicherstellung eines freien Atemwegs, effektive Beatmungen und hochqualitative Thoraxkompressionen, essenziell. Diesen Zielen sollte in den nächsten Jahren ein hoher Stellenwert in der Ausbildung sowohl von Laien als auch medizinischem Fachpersonal eingeräumt werden. Diesbezüglich sind vermehrte Forschungsanstrengungen zur Optimierung von Ausbildungskonzepten und eine gute Öffentlichkeitsarbeit zur Motivation der Laienhilfe dringend notwendig. Der Beitrag fasst die aktuell gültigen Basismaßnahmen für Laien und für Fachpersonal zusammen und gibt praxisorientierte Tipps.
Abstract
Timely initiation of resuscitation measures has an enormous impact on the outcome of a child after cardiac arrest; therefore, basic life support started by non-specialized lay rescuers as well as by professional healthcare providers plays an outstanding role within the guidelines for pediatric cardiopulmonary resuscitation (CPR). Resuscitation measures initiated by bystanders are associated with a better neurological outcome and better survival in adult and pediatric patients compared to no bystander CPR; therefore, the early recognition of a critically ill child as well as an early initiation and an effective performance of basic life support actions, such as securing an open airway, effective ventilation and high quality compressions are essential. These targets should be addressed in resuscitation education for lay people and medical professionals within the coming years. Thus, more research on optimization of educational concepts as well as public relations activities are necessary. This article summarizes the current guidelines on pediatric basic life support and provides practice-oriented recommendations.
