Zusammenfassung
Initiale Notfälle in der pädiatrischen Hämatoonkologie stellen seltene lebensbedrohliche Situationen dar, die anhand von Anamnese, Alter, Klinik, Labor und Bildgebung der Patienten erkannt werden können. Die Vermeidung bzw. fortschreitende Reduktion von initialen und therapieassoziierten Fatalitäten sind als wesentlicher Schritt in der weiteren Verbesserung der Überlebensraten von Kindern und Jugendlichen mit Krebserkrankungen zu betrachten. Im vorliegenden Beitrag werden sowohl krankheits- als auch therapieassoziierte onkologische Notfallsituationen erläutert, zu denen die Trachealkompression und obere Einflussstauung bei Mediastinaltumoren, Invaginationen und untere Einflussstauung bei abdominalen Tumoren, die Hirndrucksymptomatik bei zentralnervösen Tumoren, die Rückenmarkkompression bei Sanduhrtumoren und das Tumorlysesyndrom gehören. Die in Frage kommenden Differenzialdiagnosen an malignen Tumoren werden entsprechend der jeweiligen Lokalisation im Detail dargestellt, ebenso Art und Risiken des diagnostischen Vorgehens, welches unter allen Umständen so rasch als möglich an einem spezialisierten pädiatrischen hämatologisch-onkologischen Zentrum mit entsprechender Erfahrung erfolgen soll.
Abstract
Initial emergencies in pediatric hemato-oncology represent rare life-threatening situations which can be recognized by the combination of patient medical history, age, clinical presentation, laboratory diagnostics and imaging studies. Avoidance and the ongoing reduction of initial and therapy-related fatalities have significantly contributed to the increasing survival rate of children and adolescents with malignant diseases. In the present manuscript both disease and therapy-related oncologic emergencies will be described including tracheal collapse and superior vena cava syndrome in cases of mediastinal tumors, intussusception and inferior vena cava syndrome in cases of abdominal tumors, intracranial hypertension in brain tumors, spinal cord compression in cases of paravertebral tumors and tumor lysis syndrome. Possible differential diagnoses of malignant diseases and the type and risk of the diagnostic procedures will be shown in detail, emphasizing that all diagnostic procedures should be carried as rapidly as possible at experienced and specialized pediatric hemato-oncologic centres.
