Zusammenfassung
Hintergrund
In der Literatur wird davon ausgegangen, dass bei morbid adipösen Patienten kein langfristiger Gewichtsverlust durch selbstständiges Abnehmen möglich ist. Die Frage nach dem Stellenwert der konservativen Gewichtsreduktion ist jedoch unvollständig beantwortet.
Patienten
Zwischen 1999 und 2003 wurden an der Universitätsklinik für Plastische Chirurgie Innsbruck insgesamt 197 plastisch-chirurgische Operationen bei 120 ursprünglich morbid adipösen Patienten mit einem Ausgangs-BMI von über 35–40 und einer Gewichtsreduktion von durchschnittlich 40% ihres Körpergewichtes durchgeführt.
Ergebnisse
Bei 66% der Patienten wurde eine Magenbandimplantation vorgenommen, 34% reduzierten ihr Gewicht selbstständig. Dies war prinzipiell bei jedem Ausgangs-BMI möglich.
Schlussfolgerung
Selbstständiges Abnehmen ist für morbid adipöse Patienten mit entsprechenden mentalen Voraussetzungen prinzipiell möglich. Im Rahmen einer interdisziplinären Betreuung soll es Aufgabe des behandelnden Psychiaters sein, primär diese Patienten zu selektionieren und einem konservativen Abnehmprogramm zuzuführen. Die Chance auf Erfolg ist umso größer, je niedriger der Ausgangs-BMI liegt.
Abstract
Background
According to the literature, conservative weight loss seems to be ineffective for morbidly obese subjects. Nevertheless, the significance of nonsurgical strategies for the treatment of morbid obesity is still unclear.
Patients
From 1999 to 2003, 197 reconstructive operations were performed on 120 morbidly obese patients. Initial body mass index (BMI) was higher than 35–40, and weight loss exceeded 40% of the original body weight.
Results
Gastric banding was performed in 66% of the patients; 34% reduced their body weight by conservative means. Conservative weight loss could be achieved at each level of BMI.
Conclusion
Conservative weight loss is a valuable option for the treatment of morbid obesity in a selected group of patients. Condiolates candidates for conservative weight loss should be selected carefully by a multidisciplinary team with psychiatric expertise.
