Zusammenfassung
Hintergrund
Das Adenokarzinom des ösophagogastralen Übergangs verzeichnet in den letzten Jahren eine steigende Inzidenz. Während für das fortgeschrittene Karzinom ein multimodales Therapiekonzept etabliert wurde, ist die Therapie des Frühkarzinoms nicht eindeutig definiert.
Ziel
Es erfolgt eine Darstellung und Übersicht der endoskopischen Therapieoptionen gemäß aktueller Datenlage und Wissensstand.
Material und Methoden
Eine selektive Literaturrecherche über Pubmed, beziehend auf das Thema endoskopische Therapien des Adenokarzinoms des ösophagogastralen Übergangs, wurde durchgeführt.
Ergebnis und Diskussion
Für eine endoskopische Therapie eignen sich Patienten mit einem geringen Risiko einer nodalen Metastasierung. Die Indikation betrifft insbesondere mukosal begrenzte Karzinome (T1a) und kann in Ausnahmefällen auf Tumoren mit submukosaler Beteiligung ausgeweitet werden (T1b). Die endoskopische Therapie sollte in einem Zweistufenkonzept, bestehend aus einem resezierenden und ablativen Verfahren, durchgeführt werden. Ersteres wird mittels endoskopischer Mukosaresektion oder Submukosadissektion ausgeführt. Anschließend, falls noch residuales Barrett-Epithel besteht, wird zur Minimierung der Wahrscheinlichkeit für ein Rezidiv das Barrett-Epithel mittels einer Radiofrequenzablation oder einer Argon-Plasma-Koagulation destruiert. Bei der Durchführung einer endoskopischen Therapie durch erfahrene Endoskopiker kann so ein zur operativen Versorgung gleichwertiges onkologisches Outcome bei einer relevanten Komplikationsrate von <3 % erreicht werden. Die Komplikationen beinhalten Blutungen, iatrogene Perforationen oder Strikturen. Nach erfolgreicher endoskopischer Resektion muss eine engmaschige Nachsorge berücksichtig werden. Sollte ein Rezidiv auftreten, so kann dies erneut einer endoskopischen Therapie zugeführt werden.
Abstract
Background
The incidence of adenocarcinoma of the esophagogastric junction has increased in recent years. While a multimodal treatment concept is established for advanced carcinomas, the treatment of early stage carcinoma is still not clearly defined.
Objective
Presentation and overview of the endoscopic treatment options according to current data and knowledge.
Material and methods
A selective literature search was conducted in PubMed regarding endoscopic treatment of adenocarcinoma of the esophagogastric junction.
Results and discussion
Patients with a low risk of nodal metastasis are suitable for interventional endoscopic treatment. The indications apply particularly to carcinomas (T1a) limited to the mucosa and can in exceptional cases also apply to carcinomas with submucosal infiltration (T1b). Endoscopic treatment of these tumors should be performed in a two-step approach consisting of resection and an ablative procedure. The former is performed by endoscopic mucosal resection or endoscopic submucosal dissection. Subsequently, if residual Barrett epithelium is still present, radiofrequency ablation or argon plasma coagulation is recommended to minimize the likelihood of neoplastic recurrence. When endoscopic treatment is conducted at a large center with experienced endoscopists an oncological outcome equivalent to operative care can be achieved with a relevant complication rate <3%. The complications include bleeding, iatrogenic perforations or strictures. After successful endoscopic resection a close follow-up care must be performed. If a recurrence occurs endoscopic treatment can be repeated.
