Zusammenfassung
Aufgrund der Zunahme des Anteils älterer Menschen an der Gesamtbevölkerung steigt, wie bei anderen alters-assoziierten Erkankungen auch, die Prävalenzrate der Alzheimer-Demenz (AD) exponentiell an. Aufgrund der vielfachen Hinweise dafür, daß eine 15–30 Jahre dauernde Phase kontinuierlicher Ablagerung von senilen Plaques und neurofibrillären Bündeln (Rumble et al. 1989) der klinischen Manifestation vorausgeht, die wiederum durch eine Durchschnittsdauer von ca. 7 Jahren gekennzeichnet ist (Growdon et al. 1985), muß angenommen werden, daß lediglich präventive Therapieansätze deutliche Erfolge erbringen können. Die Umsetzung von präventiven Therapiestrategien setzt die Verfügbarkeit von validen diagnostischen biologischen Markern oder Verlaufsparametern bei Patienten mit einer sporadischen AD voraus. Auf die meisten der bisher vorgeschlagenen Proteine schloß man aus der Zusammensetzung der spezifischen Ablagerungen bei der AD, den senilen Plaques und neurofibrillären Bündeln oder dem mit der Neurodegeneration verbundenen Verlust an Synapsen. Zahlreiche Studien weisen darauf hin, daß die progrediente cerebrale Ablagerung des unlöslichen 40–43 Amisosäuren umfassenden Amyloid β-Peptids (Aβ) in Form seniler Plaques eine der Schlüsselereignisse in der Pathogenese der Erkrankung ist (Beyreuther et al. 1995). Aβ ist ein Fragment eines längeren transmembranen Vorläuferproteins (Kang et al. 1987), das Amyloid Precursor Protein (APP) genannt wird und in mehreren Isoformen vorliegt.
