Zusammenfassung

Der Teilgelenkersatz am Kniegelenk blickt auf eine lange Geschichte zurück. Bereits 1952 berichteten Elliot und McKeever sowie Macintos über die Entwicklung von Teilgelenkprothesen. Erste Publikationen zum klinischen Einsatz gingen in den 70er-Jahren auf Engelbrecht und Marmor zurück. Aufgrund verbesserter Resultate der Totalendoprothetik hatte der Teilgelenkersatz zwischenzeitlich an Bedeutung verloren. Insbesondere die höheren frühzeitigen Revisionsraten waren abschreckend für diese Verfahren. Die Verfügbarkeit neuerer Implantate mit einer besseren Reproduktion der natürlichen anatomischen Geometrie und Biomechanik sowie aktuelle Ergebnisse haben nicht erst in letzter Zeit neues Interesse für die Anwendung der Teilendoprothetik am Kniegelenk geweckt. Es bestehen klare Indikationen, bei denen mit guten klinischen Ergebnissen gerechnet werden kann. Im Vergleich zur Totalendoprothetik ist von einer schnelleren Rehabilitation mit kürzerer Krankenhausdauer und reduzierter Morbidität, einem besseren Bewegungsausmaß, besserer Funktion und höherem Aktivitätslevel sowie weniger perioperativen Komplikationen auszugehen. Demgegenüber steht weiterhin eine erhöhte Revisionsrate in den ersten Jahren nach der Implantation. Dieses Kapitel gibt einen Überblick über Indikationen, operative Techniken und Ergebnisse nach Teilgelenkersatz am Kniegelenk.
