Zusammenfassung
Artefakte sind störende künstliche Produkte, die vor allem am Thorax aufgrund der besonderen anatomischen Verhältnisse die Bildgebung und Beurteilung erheblich erschweren. Andererseits ermöglicht das Fehlen sonst typischer Artefakte an der Lungenoberfläche oder am Knochenthorax die Diagnose bestimmter Erkrankungen (subpleurale Lungenläsion, Rippenfraktur), da dann die Beurteilung des Parenchyms oder der Knochen möglich wird. Schließlich dienen auch Artefakte als diagnostisches Kriterium, wie z. B. Luft mit Nachhall-Echos im Pleuraraum bei Pneumothorax. Eine besondere klinische Bedeutung haben die so genannten B-Linien (siehe auch Kapitel 4) bei der Diagnose des interstitiellen Syndroms, insbesondere bei Patienten mit akuter Dyspnoe (Notfallsonographie, Notaufnahme, Intensivstation). Werden diese Artefakte direkt aus der ansonsten intakten Pleurareflexlinie in mindestens 2 Interkostalbereichen pro Seite nachgewiesen, erlaubt dies die Differentialdiagnose des interstitiellen Syndroms (häufig interstitielles Ödem=Hyperhydratation, viel seltener entzündliche interstitielle Veränderungen, Kontusion oder chronisch fibrotische Veränderungen) und die Abgrenzung zu anderen Ursachen der Dyspnoe (z.B. exazerbiertes Asthma bronchiale). CEUS kann die B-Bild-Artefakte auch in der Thoraxsonographie nicht re.
