Zusammenfassung
Die qualitative Color-Doppler -Sonographie (CDS) zeigt unterschiedliche und teilweise charakteristische Befunde bei verschiedenen Arten von Lungenkonsolidierungen und ist daher eine wertvolle Ergänzung zum B-Mode-Ultraschall im Hinblick auf die ätiologische Einordnung peripherer Lungenläsionen. Entsprechend der physiologischen Doppelvaskularisation der Lunge durch die Lungenarterien und die Bronchialarterien kann man im CDS zwischen arteriellen Hochimpedanz-Spektralkurven und arteriellen Niederimpedanz-Spektralkurven im konsolidierten Lungengewebe unterscheiden. Erstere werden den Pulmonalarterien und letztere den Bronchialarterien zugeordnet. Innerhalb der peripheren Lungenkonsolidierungen zeigen verschiedene Entitäten charakteristische Verteilungsmuster der pulmonalarteriellen und bronchialarteriellen Flusssignale in Bezug auf Frequenz und Ort. Die Analyse der arteriellen Spektralkurven ist zeitaufwändig. Eine zuverlässige Abgrenzung von Gefäßen der Tumor-Neoangiogenese ist mit CDS derzeit nicht möglich. Das orientierende CDS wird nur in der täglichen klinischen Routine eingesetzt, aber die Spektralkurvenanalyse ist oft zu zeitaufwendig und störanfällig.
Die Erfahrungen mit CEUS bei peripheren Lungenläsionen sind begrenzt. Lungenläsionen können durch die Zeit bis zum Beginn der Kontrastmittelanreicherung sowie durch das Ausmaß und die Homogenität der Kontrastmittelanreicherung beschrieben werden. Kontrastmittelverstärkter Ultraschall am Brustkorb kann einfach und schnell durchgeführt werden und ist daher grundsätzlich für den klinischen Routineeinsatz geeignet. Die möglichen Indikationen sind 1) im Bereich der Thoraxwand die Differenzierung von vitalem und avitalem Gewebe sowie die Tumorcharakterisierung; 2) beim Leitsymptom Thoraxschmerz ist der CEUS hilfreich bei der Differentialdiagnose von Pleuritis und peripherer Lungenembolie; 3) beim Leitsymptom der Röntgenverschattung ist die CEUS hilfreich bei der Diagnose des malignen Pleuraergusses (PE) und des infarktbedingten PE sowie bei der Unterscheidung von Tumor und zentraler Atelektase, beim Nachweis atypischer Lungenprozesse und bei der Beurteilung der Bösartigkeit von Lungenknoten oder peripheren Bronchialkarzinomen. Im Bereich der interventionellen Sonographie ermöglicht die CEUS eine sichere Unterscheidung zwischen Tumor, Nekrose und Atelektase.
