Zusammenfassung
Die Schizophrenie zählt aufgrund des frühen Ersterkrankungsalters, den häufig chronischen Verläufen und den Beeinträchtigungen in den Lebensbereichen Wohnen, Arbeit und Sozialbeziehungen unter den pychischen Erkrankungen zu den Störungsbildern mit hohen Pro-Kopf-Versorgungskosten. Trotz der hohen Budgetrelevanz haben gesundheitsökonomische Untersuchungen der verschiedenen Behandlungs- und Versorgungsformen im Bereich der Schizophrenie keine lange Tradition. In Deutschland wurden sie erst Mitte der 1990́er-Jahre zu einem kontinuierlichen Forschungsgegenstand, der jedoch auch gegenwärtig immer noch von nur wenigen Forschungsgruppen bearbeitet wird. Der Forschungsoutput ist entsprechend gering. Befunde über die Kosten und die Kosteneffektivität der Behandlung der Schizophrenie sind stets vor dem Hintergrund der jeweils implementierten psychiatrischen Versorgungsstruktur und den geltenden Finanzierungsmechanismen zu sehen und zu interpretieren. Länderspezifische Charakteristiken wie die allgemeine Organisation des Gesundheitswesens, Finanzierungsweisen und Entgeltverfahren, Verfügbarkeit einer Krankenversicherung, Lohn- und Preisgestaltung, Ausbaugrad und Integration der psychiatrischen Versorgung usw. haben einen entscheidenden Einfluss auf die Kostenentwicklung. Sowohl international als auch in Deutschlang können regional abweichende Kontextfaktoren gesundheitsökonomische Befunde entscheidend beeinflussen. Zudem beruhen empirische Kostendaten auf dem Preisniveau des jeweilig untersuchten Zeitraums und müssen beim Kostenvergleich über verschiedene Studien oder Zeiträume hinweg entsprechend angepasst werden. Demnach können Ergebnisse aus internationalen Gesundheits- und Versorgungssystemen nicht einfach auf innerdeutsche Verhältnisse übertragen werden. Selbst ein Vergleich innerdeutscher Befunde muss mit hoher methodischer Sorgfalt erfolgen.
