Zusammenfassung
Thorakale Verletzungen sind klinisch relevant und potenziell letal. Sie sind häufige Begleitverletzungen (45 %) bei Polytraumatisierten und können einen wesentlichen prognostischen Faktor darstellen. Gefährdet sind knöcherne Strukturen, der pleurale Raum, die Lunge und das tracheobonchiale System, wie auch das Herz und die großen mediastinalen Gefäße. 80–90 % der Thoraxtraumen lassen sich durch Sicherung des Atemwegs, suffiziente Oxygenierung und ggf. Anlage einer oder mehrerer Thoraxdrainagen suffizient therapieren. Operative Interventionen sind vergleichsweise selten indiziert und im Fall von Notfallthorakotomien mit einer sehr hohen Letalität belastet. In Westeuropa sind wir therapeutisch selten mit penetrierenden Verletzungen konfrontiert (unter 10 %). In Mittel- und Südamerika, Afrika oder Osteuropa sind penetrierende Schuss- und Stichverletzungen deutlich häufiger.
