Zusammenfassung
Im Zusammenhang mit der nicht-pharmakologischen Behandlung depressiver Syndrome wurden verschiedene Veränderungen des Schlaf-Wach-Rhythmus beschrieben (zur Übersicht: Kuhs u. Tölle 1986). Dabei ist hervorhebenswert, daß für alle der dabei aufgeführten Formen, außer der des partiellen Schlafentzugs der ersten Hälfte der Nacht, eine antidepressive Wirksamkeit belegt werden konnte. Die einzelnen therapeutischen Veränderungen des Schlaf-Wach-Rhythmus sind in Tabelle 1 dargestellt. Die am meisten angewandte und am besten untersuchteste Methode ist der totale Schlafentzug, bei der der Patient angehalten wird, die gesamte Nacht und den darauffolgenden Tag wach zu bleiben (36–40 Std.). Beim partiellen Schlafentzug der ersten Hälfte der Nacht, ist der Patient z. B. bis 2 Uhr morgens wach und schläft anschließend. Beim partiellen Schlafentzug der 2. Hälfte der Nacht kann dahingegen der Patient von z. B. 21 Uhr abends bis 2 Uhr morgens schlafen und wird anschließend angehalten wach zu bleiben. Weiterhin wird noch die selektive Unterdrückung des REM-Schlafes (Vogel et al. 1968, 1980) sowie die „Phase-Advance“-Therapie (Wehr et al. 1979; Sack et al. 1985; Souetre et al. 1987) als eine mögliche therapeutische Veränderung des Schlaf-Wach-Rhythmus beschrieben. Bei der Phase-Advance-Therapie ist die Schlafperiode über einen längeren Zeitraum (z. B. 2 Wochen) im Tagesrhythmus vorverschoben, d. h. der Patient schläft z. B. von 18 Uhr abends bis 2 Uhr morgens und ist anschließend wach.
