Zusammenfassung
Der Lidapparat ist eine Schutzeinrichtung für das Auge, wobei die Befeuchtung der Hornhaut eine zentrale Rolle spielt (Abb. 1). Wenn bei Kaninchen der Tränenfilm an der kornealen Oberfläche abgewaschen wird, nimmt der Korneadurchmesser sofort ab (Abb. 2). Wird die Korneaoberfläche mit dem Sekret der Meibom-Drüsen überzogen, bleibt die Korneadicke unverändert, auch wenn die Lider offenbleiben (1). Diese Versuche zeigen, daß der präkorneale Tränenfilm das Austrocknen der Kornea verhindert und daß die oberflächliche Lipidschicht hierfür eine wesentliche Rolle spielt. Wird bei Kanin-chen die Tränendrüse entfernt, kommt es zu einer Keratinisation des Hornhautepithels. Dies zeigt, daß auch die wäßrige Phase für die Funktionsfähigkeit der Hornhaut von Bedeutung ist. Die Haftung der Flüssigkeitslamelle auf der Hornhautoberfläche setzt aber die Existenz einer Muzinschicht voraus, die an den Oberflächenzellen des Korneaepithels, deren Oberfläche durch feinste Mikrovilli und Mikroplicae stark vergrößert ist, klebt. Fehlt diese Schleimschicht, die von den Drüsen der Konjunktiva (hauptsächlich den Becherzellen) produziert wird, haftet der Tränenfilm nicht mehr am Epithel und die Kornea wird geschädigt. Der präkorneale Tränenfilm hat also drei verschiedene Komponenten, die aus unterschiedlichen Quellen stammen und erst in ihrer Gesamtheit ein harmonisches, die Hornhautfunktion erhaltendes System abgeben (Abb. 3).
