Zusammenfassung
92 Patienten mit folgenden Erkrankungen wurden skelettszintigraphisch untersucht: Psoriasis vulgaris Pv (n = 39), Psoriasis inversa P inv (n = 12), Psoriasis pustulosa — Typ Königsbeck-Barber Pp K-B (n = 20) Pustulosis palmaris et plantaris Ppp (n = 21), und ein Kontrollkollektiv K (n = 12). Bei allen Patienten wurden Skelettszintigraphien angefertigt. In jedem Fall erfolgte die Untersuchung von 73 Gelenken. Diese wurden in 3 Gelenkgruppen unterteilt:

Gelenkgruppe (periphere Gelenke) – 60 Gelenke (bzw. Gelenkgruppen)

2.
Gelenkgruppe (große Gelenke) – 8 Gelenke, sowie eine
-
3.
Gelenkgruppe (Stammskelett) – 5 Gelenkgruppen.
Bei 92 Patienten wurden (92×73) 6716 Gelenke untersucht. Von diesen zeigten 1059 (15,7%) eine vermehrte Anreicherung des Technetium-markierten Methylendiphosphonats. Bei den Patienten des Kontrollkollektivs (n = 12) fanden sich bei 12×73 = 876 Gelenken bzw. Gelenkgruppen 29× (3,3%) Tracerablagerungen.
Bei Patienten mit Pv stellten sich durchschnittlich 17,1% der Gelenke szintigraphisch auffällig dar, bei Patienten mit P inv 17,0%, mit Pp K-B 7,6% und mit Ppp 5,7%. Beim Kontrollkollektiv fanden wir fokale Aktivitäten bei 2,4% der untersuchten Skelettabschnitte. Auf die Gelenkgruppe 1 (5520 Gelenke) entfielen 970 Aktivitätszonen, auf die Gelenkgruppen 2 und 3 zusammen 89.
Somit werden fokale Aktivitäten bei der Psoriasis vulgaris und der Psoriasis inversa 3 mal so häufig wie bei der Pustulosis palmaris et plantaris und etwa 7 mal so häufig wie beim Kontrollkollektiv gefunden. Die Unterschiede zentrieren sich brennglasartig auf die peripheren Gelenke.
Folgende Schlüsse lassen sich ziehen:

die Pustulosis palmaris et plantaris und die Psoriasis pustulosa Typ Königsbeck-B arber weisen signifikant häufigere Aktivitätsanreicherungen des Skeletts auf als ein nicht erkranktes Kontrollkollektiv. Den Aktivitätszonen muß demnach eine pathogenetische Bedeutung beigemessen werden.

2.
Die Pustulosis palmaris et plantaris und die Psoriasis pustulosa Typ Königsbeck-Barber unterscheiden sich in der Frequenz des Skelettbefalls von der Psoriasis vulgaris und der Psoriasis inversa.

3.
Das charakteristische psoriatische Befallmuster mit transversalem und axialem Befall wird bei allen Patientenkollektiven angetroffen.
Summary
92 patients with the following diseases were examined by bone scintigraphy; psoriasis vulgaris Pv (n = 39), psoriasis inversa P inv (n = 12), psoriasis pustulosa of the Königsbeck-Barber type PpK-B (n = 20), pustulosis palmaris et plantaris Ppp (n = 21) and a collective of healthy persons K (n = 12). In all patients bone scintigraphy was performed. In each person 73 joints were examined, that were divided in three groups:

group 1
(peripheral joints) — 60 joints
-
group 2
(big joints) — 8 joints
-
group 3
(joints of the skeleton of the trunc) — 5 joints.
In 92 patients (92×73) 6716 joints were examined. An increased uptake-ratio was found in 1059 joints (15,7%). In the control group accumulations of the radiotracer were found 29× (3,3%). In patients suffering from psoriasis vulgaris a pathological radionuclid uptake could be shown in 17,1% of the examined joints; a pathological tracer uptake was found in patients with P inv in 17,0% of the examined joints, in Pp K-B in 7,6% and in Ppp in 5,7%. In controls focal activities were seen in 2,4%. In peripheral joints (5520 joints) 970 areas of pathological radionuklid uptake were found, in the group 2 and 3 together 89 pathological foci. Focal activities were found in psoriasis vulgaris and in psoriasis inversa three times as much as in pustulosis palmaris et plantaris and about seven times as much as in the controls. The differences are centred in the peripheral joints.
It can be concluded that:

in the pustulosis palmaris et plantaris and in the psoriasis pustulosa of the Königsbeck-Barber type skeletal focal activities were found in a higher percentage as in controls.

2.
the pustulosis palmaris et plantaris and the psoriasis pustulosa of the Königsbeck-Barber type differ in the frequency of skeletal uptake of the radiotracer from patients with psoriasis vulgaris or psoriasis inversa.

3.
the characteristical psoriatic pattern with axial and transversal joint-involvement was found in all groups of patients.
