Szintigraphische Befunde bei progressiver Sklerodermie

Zusammenfassung

Die progressive Sklerodermie (PSS) manifestiert sich häufig an den Gelenken, deren Darstellung im Szintigramm jedoch bislang nicht systematisch untersucht worden ist. Daher wurden bei 35 Patienten Ganzkörperszintigramme und Einzelszintigramme des Hand- und Fußskeletts mit ^99mTechnetium-Methylendiphosphonat durchgeführt.

Das Kollektiv I bestand aus 19 Patienten mit einer PSS, das Kollektiv II aus 10 Patienten mit einer Morphaea und das Kollektiv III aus 6 Patienten mit alleiniger Raynaud-Symptomatik. Innerhalb des Kollektivs mit einer PSS waren die 8 großen Gelenke des Bewegungsapparates zwischen 36,8% und 47,4% befallen, die kleinen Gelenke des Handskeletts waren zu 49,8% (rechts) und zu 44,2% (links) befallen, während das Stammskelett nur bei einer Patientin befallen war. Im Bereich des Handskeletts waren MCP-, PIP- und DIP-Gelenke gleichermaßen befallen. Dieses szintigraphisch sichtbare charakteristische Befallmuster der Polyarthritis der PSS dient zur differentialdiagnostischen Abgrenzung besonders zur chronischen Polyarthritis, so daß die Skelettszintigraphie zur Frühdiagnose einer PSS beitragen kann.

Bei einzelnen Patienten mit einer Morphaea und alleiniger Raynaud-Symptomatik wurden ebenfalls Aktivitätsanreicherungen besonders im Handskelett gefunden.

Summary

There ist no exact investigation of the value of bonescintigrafy to detect the frequent articular manifestations in progressive systemic sclerosis (PSS). Therefore we examined 35 patients with a whole body scintigram and a special bone-scintigram of the hands and feet with ^99mtechnetium-methylendiphosphonate. Group I consisted on 19 patients with a PSS, group II consisted on 10 patients with a morphaea and group III consisted on 6 patients suffering only from Raynaudsyndrome.

The 8 large joints were affected in the group I with a frequence from 36,8% to 47,8%, the small joints of the bones in hands and feet were involved in 49,8% on right side and 44,2% on left side, whereas the trunk skeleton was affected in only one case. The MCP-, PIP- and DIP-joints were involved in about the same frequence.

There is a characteristic form of manifestation of polyarthritis in PSS demonstrated by bonescintigrafy, this helps to differentiate polyarthritis in PSS from rheumatoide polyarthritis and may contribute to an earlier diagnosis of PSS.

In some cases of morphaea and Raynaud-syndrome there was especially in the joints of the hands an increased accumulation of 99 m technetium-diphosphonates.
