Zusammenfassung

Die Untersuchungen von Winberg haben überzeugend belegt (Winberg et al. 1974), daß die Pyelonephritis eine „typische Kinderkrankheit“ ist: Bei Jungen ereignen sich die Mehrzahl der Pyelonephritiden im 1. (und 2.) Lebensjahr, bei Mädchen im 1. (bis 5.) Lebensjahr (Abb. 7.1). Zahlreiche andere Untersuchungen haben dazu beigetragen, die Konzentration der Pyelonephritis auf die ersten Lebensjahre verständlich zu machen. So wissen wir heute, daß der vesikoureterorenale Reflux (VUR) im Säuglingsalter häufiger als danach vorkommt und daß die stärksten Refluxgrade in diesem Lebensabschnitt zu finden sind (International Reflux Study Committee 1981; Olbing 1987; Smellie et al. 1975). Angeborene Obstruktionen im harnableitenden System sind in diesem Alter besonders ausgeprägt und unkorrigiert. Die periurethrale Kontamination mit Bakterien wird durch die Windelperiode begünstigt und bei Jungen mit belassener Vorhaut kommt es in den ersten Lebensmonaten deutlich häufiger zu symptomatischen Harnwegsinfektionen als bei Säuglingen nach Zirkumzision (Ginsberg 1982; Wiscoll et al. 1985; Svanborg et al. 1978; Winberg et al. 1982).
