Zusammenfassung
Harnwegsinfektionen stellen die am häufigsten vorkommenden bakteriellen Infektionen in Industriestaaten dar (Kunin 1997). In Deutschland werden jährlich ca. 2 Mio. Arztbesuche gezählt, bei denen Harnwegsinfektionen im Mittelpunkt der Konsultationen stehen (Hacker 1996). Hinsichtlich des Typs der Infektionen muß zwischen unkomplizierten, d. h. Infektionen bei Patienten, bei denen der Harnweg keine Abnormalität aufweist, und komplizierten Infektionen unterschieden werden. Bei unkomplizierten Harnwegsinfektionen werden bis zu 80% der Fälle durch E. coli-Bakterien ausgelöst. Weiterhin haben 20–40% der komplizierten Harnwegsinfektionen ebenfalls ihre Ursache in E. coli-Keimen. Diese uropathogenen E. coli-Bakterien unterscheiden sich von Durchfallserregern, aber auch von normalen fäkalen E. coli-Keimen durch die Produktion bestimmter Virulenzfaktoren (Mühldorfer u. Hacker 1994; s. unten und Abb. 4.1a). In der Regel stellen die harnwegsinfizierenden Mikroorganismen ehemals fäkale Stämme desselben Patienten dar, die durch die aszendierende Route zunächst in die Blase und dann ggf. in die Niere gelangen. Epidemiologische Untersuchungen haben gezeigt, daß ein kleiner Teil der Darmflora (ca. 10% der fäkalen E. coli-Bakterien) potentiell uropathogene Keime darstellen (Johnsen 1991). Diese Keime können dann unter bestimmten Bedingungen (Veränderung des Wirtsstatus) eine Infektion auslösen.
