Zusammenfassung
Glukokortikoide werden physiologischerweise in der Zona fasciculata der Nebennierenrinde synthetisiert. Die Ausgangssubstanz ist Cholesterol, das teilweise in der Nebennierenrinde selbst synthetisiert und teilweise von außen aufgenommen wird. Wichtige Zwischenstufen der Kortisolsynthese sind Pregnenolon und Progesteron. Aus Progesteron können neben Kortisol auch Aldosteron und Testosteron synthetisiert werden. Die Steroidsynthese wird gesteuert durch eine Reihe von Cytochrom P 450 Isoenzymen. Das adrenokortikotrope Hormon (ACTH), das von der Hypophyse synthetisiert wird, aktiviert die Proteinkinase C, die wiederum die Synthese verschiedener Cytochrom P 450 Isoenzyme induziert. Außerdem wird die Sekretion in der Nebennierenrinde durch einen negativen Feed-Back-Mechanismus gesteuert. Das gemeinsame Grundgerüst aller genannten Hormone ist der Phenanthrenring, dem eine Zyklopentangruppe angegliedert ist und das damit einen C17 Körper bildet. Die Strukturvoraussetzung für eine Glukokortikoidwirkung sind eine Ketogruppe in Stellung C3 und C20, eine Hydroxylgruppe in Stellung Cii, C17 und C21 und eine Doppelbindung zwischen C4 und C5 (Abb. 12.1).
