Zusammenfassung
Die Nachbehandlung nach einer Elektrokonvulsionstherapie ist von außerordentlicher Bedeutung. Ohne diese beträgt die Rückfallrate bei depressiven Störungen innerhalb der ersten 6 Monate über 80 %. Bei der unipolaren depressiven Störung ist die Erhaltungstherapie mit Lithium und Nortriptilin effektiv, alternativ kann auch eine Erhaltungs-EKT (EEKT) angewandt werden. Nach einem Rückfall bei adäquater Pharmakotherapie ist eine EEKT indiziert. Bei der depressiven Episode im Rahmen der bipolaren Störung sollte eine EEKT dann empfohlen werden, wenn eine medikamentöse Erhaltungstherapie nicht ausreichend war. Bei der schizophrenen und der schizoaffektiven Störung ist eine EEKT nach erfolgreicher EKT optional, nach der 2. EKT-Serie zu empfehlen. Sie sollte immer mit einer antipsychotischen Medikation kombiniert werden. Die Frage nach der Frequenz der Behandlung im Rahmen der EEKT ist noch offen, eine bedarfsorientierte Gabe ist jedoch gegenüber einem festen Schema zu bevorzugen. Eine Psychotherapie ist in jedem Fall zu empfehlen.
