Zusammenfassung
Therapeutische excimerchirurgische Eingriffe werden unter der Abkürzung PTK zusammengefasst, die für phototherapeutische Keratektomie steht. Gemeint ist hiermit die Entfernung oberflächlicher, pathologisch veränderter Hornhautschichten mit dem Excimerlaser aus unterschiedlichen Intentionen heraus [11]: Die Ablation oberflächlicher Narben dient meistens der Visusverbesserung (◘ Abb. 11.1), kann aber auch nur für einen verbesserten Kontaktlinsensitz förderlich sein. Die Glättung kornealer Irregularitäten durch eine PTK führt über die verbesserten Brechungseigenschaften der Kornea auch zu einer Anhebung der Abbildungsqualität des Auges insgesamt und lässt den Visus ansteigen. Ein ganz anderes Ziel hat die teilweise Ablation der Bowman-Membran bei rezidivierenden Erosionen: Hier soll möglichst refraktionsneutral eine verbesserte Haftung des Epithels an epithelialer Basalund Bowman-Membran über einen Anstieg der Dichte der Hemidesmosomen bewirkt werden.
