Abstract
The extraperitoneal access to robotic radical prostatectomy duplicates best the open retropubic technique. Avoiding the abdominal cavity, the technique excludes potential intraperitoneal complications. Total operative time is shorter. Postoperative subileus is reduced and return to full diet earlier. Urinary leaks and postoperative bleedings may treated conservatively. For extended lymphadenectomy and prostatectomies after laparoscopic mesh implant, however, a transperitoneal technique is favorable; therefore, training in both access techniques, extra- and transperitoneal, should be carried out.
