Komorbidität ist ein Begriff, der erst in den 90er Jahren des letzten Jahrhunderts auftaucht und eng mit der neuen diagnostischen Nomenklatur der ICD-10 und der DSM-IV zusammenhängt. Typischerweise ist der Begriff deshalb auch z.B. im Lexikon der psychologischen Begriffe (Peters 1990) noch nicht enthalten. Der Grand für die Notwendigkeit des Begriffes liegt in der Abkehr vom klassischen, durch Ideologic begründeten, ätiologischen Krankheitskonzept, d. h. der Abkehr von Kategorien und Hinwen-dung zu Dimensionen. Die neue ICD-10 erhält entsprechend phänomenologische Diagnosebeschreibungen, die den Vorteil haben, dass sie unter unterschiedlichen Bedingungen, auch im transkulturellen Kontext, stabil wiedererkannt werden können. Aus diesem phänomenologischen Diagnostikkonzept ergibt sich logisch das Nebeneinander von Diagnosen oder Krankheitsbildern, die früher in komplexen Krankheitskonzepten gemeinsam enthalten waren. Das klassische Beispiel ist die Differenzierung zwischen depressiven Erkrankungen einerseits und Angsterkrankungen andererseits. Noch in den 70er Jahren des letzten Jahrhunderts wurde intensiv diskutiert, ob die Angstneurose sich sauber von der depressiven Erkrankung trennen lasse und ob sie sinnvollerweise in unterschiedliche Angststörungen unterteilt werden solle (McNair et al 1978).
