Insbesondere in der frühen Kindheit ist die Abgrenzung zwischen depressiven Stimmungsstörungen und Angststörungen noch sehr schwierig, so dass oftmals der Terminus affektive Störungen für beides, Angst und depressive Phänomene, benutzt wird, weil beide Störungen der Affektivität beinhalten. Affektive Störungen mit depressiven und Angstsymptomen kommen dabei bei Kindern und Jugendlichen durchaus häufig vor, stellen sich oftmals wiederholt in der Entwicklung ein und tendieren dazu, chronisch zu verlaufen und bis ins Erwachsenenalter zu persistieren. Die Prävalenz solcher Störungen wird sehr unterschiedlich angegeben: US-amerikanischen Schätzungen gehen dahin, dass von ungefähr 2% aller Kinder vor der Pubertät und 4% aller Adoleszenten zu irgendeinem Zeitpunkt ihrer Entwicklung unter Depressionen leiden (Fleming & Offord, 1990). Dabei seien Jungen und Mädchen in der Kindheit gleich betroffen, wogegen in der Adoleszenz zweimal so viele Mädchen an dieser Stöning erkranken (Lewinsohn, Clarke, Seeley, & Rohde, 1994). Die Abgrenzung zwischen schweren, mittelschweren und leichten depressiven Storungen, Depressionen mit rezidivierendem Verlauf und anhaltenden affektiven Störungen (Dysthymia) ist im Kindes- und Jugendalter schwierig. Manchmal wird von einer Komorbidität („doppelte Depression“) gesprochen, wobei es sich doch meist um ein Krankheitsbild und nicht um das Zusammentreffen verschiedener Krankheitsbilder handelt.
