Zusammenfassung
Die heutigen Möglichkeiten, auf der Basis der sehr unterschiedlichen pharmakologischen Eigenschaften der etwa 30 zurzeit in Deutschland zur Verfügung stehenden Antidepressiva eine möglichst relevante Auswahl eines bestimmten Präparats für einen bestimmten Patienten zu finden, sind noch begrenzt. Substanzen mit dualem Wirkmechanismus, die gleichzeitig serotonerge und noradrenerge Mechanismen beeinflussen, werden heute bei schweren melancholischen Depressionen, dem früheren endogenen Typ, empfohlen. Dualer Wirkungsmechanismus gilt natürlich auch für viele alte trizyklische Antidepressiva. Obwohl diese heute noch in Deutschland stark verordnet werden, geht die weltweite Tendenz eher dahin, den TZA nur noch Reservecharakter zuzuordnen, da die zum Teil erheblichen unerwünschten Arzneimittelwirkungen und ihre hohe Toxizität ihren Einsatz als Primärtherapeutika nicht mehr rechtfertigen. Während die pharmakologischen Eigenschaften der Antidepressiva als Auswahlkriterium für die therapeutische Wirkung nur begrenzte Aussagekraft haben, spielt die Pharmakologie über die damit verbundenen unerwünschten Arzneimittelwirkungen bei der Auswahl eines individuellen Antidepressivums für einen bestimmten Patienten eine sehr viel wichtigere Rolle. Damit ist es heute auch für die neuen Substanzen möglich, eine nebenwirkungsgeleitete Auswahl des für den jeweiligen Patienten am besten geeigneten Antidepressivums zu treffen.
