Zusammenfassung

Gastrointestinale Stromatumoren (GIST) sind die häufigsten mesenchymalen Tumoren des Gastrointestinaltraktes. Entscheidendes diagnostisches Kriterium ist der Nachweis einer KIT-Expression (CD117). Trotz aller Fortschritte in der medikamentösen Therapie stellt die komplette chirurgische Resektion (R0) nach wie vor die Standardbehandlung resektabler GIST dar und ist Voraussetzung für einen kurativen Therapieansatz. Die Datenlage zum Stellenwert laparoskopischer Resektionsverfahren ist derzeit noch unzureichend. Unter dem Aspekt der minimalen Invasivität können insbesondere kleine GIST des Magens eine geeignete Indikation für ein chirurgisch-laparoskopisches Vorgehen darstellen. Grundsätzlich sollten laparoskopische GIST-Resektionen aber nur von interdisziplinären Teams aus laparoskopisch, endoskopisch und onkologisch gleichermaßen erfahrenen Chirurgen und Gastroenterologen vorgenommen werden.

Abstract

Gastrointestinal stromal tumors (GIST) are the most frequent soft tissue tumors of the gastrointestinal tract. The demonstration of KIT expression (CD117) defined GIST as a distinct tumor entity. Despite many improvements in the medical therapy, complete tumor resection remains the standard treatment for resectable GIST and is the precondition for cure. Data concerning the role of laparoscopic surgery are sparse, but small gastric GIST (<5 cm) could be a good indication for laparoscopic procedures. In principle laparoscopic GIST resections should be performed only by interdisciplinary teams of surgeons and gastroenterologists experienced in laparoscopy, endoscopy, and oncology.
