Zusammenfassung
Zunehmendes mütterliches Alter ist ab etwa 35 Jahren ein unabhängiger und direkter Risikofaktor für die Abnahme sowohl der Quantität als auch der Qualität der Eizellen, d. h. für sinkende Fekundität, eine Erhöhung der Fehlgeburtsrate sowie der kindlichen Chromosomenstörungen bei Geburt und signifikanter ab etwa 40 Jahren für Gestationsdiabetes, Placenta praevia, Sectio caesarea und intrauteriner Fruchttod. Zusammen mit anderen altersabhängigen Einflussfaktoren wirkt sich das Alter der Mutter – verstärkt ab etwa 40 Jahren – auf die erhöhte Prävalenz von Präeklampsie, Abruptio placentae, Geburtseinleitung und operative Entbindung, Mehrlingsschwangerschaft, Frühgeburtlichkeit und Wachstumsverzögerungen aus. Zum einen sollten aus den oben genannten Gründen Jugendliche bzw. junge Erwachsene über diese Risiken aufgeklärt werden, damit sie eine „informierte Entscheidung“ treffen können, wann sie den Kinderwunsch in ihrer Lebensplanung verwirklichen möchten. Zum anderen ist es wichtig, ältere Frauen mit Kinderwunsch zu beraten, frühzeitig präventive gesundheitliche Maßnahmen zu ergreifen. Frauen über 35 Jahre, die vor der Schwangerschaft gesund sind, frühzeitig alle Vorsorgeuntersuchungen wahrnehmen und im Bedarfsfall medizinisch optimal betreut werden, haben gute Chancen, eine komplikationslose Schwangerschaft zu erleben und ein gesundes Kind zur Welt zu bringen, auch wenn einige Risiken altersbedingt erhöht sind.
Abstract
Advanced maternal age greater than 35 years is an independent and direct risk factor for declining egg quantity and quality, i.e. for decreasing fecundity, an increase in spontaneous abortion, chromosomal abnormalities in the child and more pronounced at approximtely 40 years of age, for gestational diabetes, placenta previa, cesarean section and intrauterine fetal death. In association with other age-dependent confounders, maternal age with a more pronounced effect over 40 years influences the increased prevalence of preeclampsia, placental abruption, induction of labor, operative delivery, multiple gestation, preterm birth and intrauterine growth restriction (IUGR). For these reasons teenagers and young adults have to be educated about these risks to enable them to make an informed decision on when to plan childbearing in their lives. On the other hand it is important to counsel women over 35 years old who wish to have a child about early preventive health measures. Women over 35 years who are healthy before pregnancy and who undergo early prenatal care and if necessary ideal medical treatment have a good chance of experiencing a pregnancy without complications and a good birth outcome even if there are some higher age-related risks.
