Zentrales anticholinerges, malignes neuroleptisches und Serotoninsyndrom

Wichtige Differenzialdiagnosen bei postoperativen Bewusstseinsstörungen

Zusammenfassung

Bewusstseinsstörungen sind nach Allgemeinanästhesien ein häufiges Phänomen. Neben den klassischen Ursachen (z. B. Überhang von sedierenden Medikamenten) kann eine Bewusstseinsstörung auch eine unerwünschte Reaktion auf ein Medikament sein. Viele Medikamente in der Anästhesie können diese auslösen. Alkaloide wie das Atropin können z. B. ein zentrales anticholinerges Syndrom (ZAS) auslösen, Opioide das Auftreten eines Serotoninsyndroms (SeS) begünstigen, und die Gabe eines Neuroleptikums kann zu einem malignen neuroleptischen Syndrom (MNS) führen. Diese 3 Syndrome sind aufgrund ihrer individuell sehr heterogenen Symptomatik nur schwer zu diagnostizieren. Gemeinsame Symptome wie Bewusstseinsstörung, Tachykardie, Hypertonie und Fieber erschweren die Unterscheidung zwischen den Syndromen zudem. Das klinische Spektrum kann von milden bis hin zu lebensgefährlichen Verläufen reichen. Je nach Ausprägung kann sich die Therapie auf das Absetzen des Auslösers und eine klinische Überwachung begrenzen, oder es kann eine spezifische Therapie erforderlich sein. Als spezielle Therapien werden für das ZAS Physostigmin, für das SeS Cyproheptadin und für das MNS Dantrolen empfohlen.
