Welches Biologikum/„small molecule“ in welcher Situation bei chronisch-entzündlichen Darmerkrankungen?

Zusammenfassung

Die TNF‑α-Blocker haben vor 20 Jahren die Behandlung der chronisch entzündlichen Darmekrankungen (CED) revolutioniert. In den letzten Jahren ist eine Reihe neuer Medikamente zur Behandlung der CED zugelassen worden und sie haben die Therapieoptionen wesentlich erweitert. Da in der Realität bei MC(Morbus Crohn)- und CU(Colitis ulcerosa)-Patienten häufig auch extraintestinale Manifestationen wie auch weitere Komorbiditäten auftreten, benötigt es zur Auswahl der optimalen Therapie der CED-Patienten ein individualisiertes Vorgehen. Die Lebensumstände, wie Alter, Schwangerschaft oder geplante Schwangerschaft, müssen beim Therapieentscheid berücksichtigt werden. Ältere Patienten stellen mit ihren Komorbiditäten sowohl in der Behandlung als auch bei der Diagnosestellung eine Herausforderung dar. In Zukunft wird man bezüglich der Verabreichungsform auch vermehrt die Wünsche der immer besser informierten Patienten berücksichtigen müssen.
