Verhaltenstraining als zusätzlicher Therapieansatz bei rheumatoider Arthritis

Zusammenfassung

Hintergrund

Trotz aller Erfolge der Basistherapeutika bleibt die rheumatoide Arthritis eine chronische Erkrankung, die bei vielen Patienten mit relevanten Schmerzen verbunden ist. Daher erscheint es sinnvoll, Patienten auch mit psychoedukativen Maßnahmen zu unterstützen, um Schmerz und Angst zu verringern. Von den vielfältigen psychotherapeutischen Verfahren sind nur wenige ausreichend auf ihre Wirksamkeit untersucht.

Ziel der Arbeit

Ziel ist eine Einführung und Abgrenzung verhaltenstherapeutischer und achtsamkeitsbasierter Trainingsmethoden zur Verbesserung der Resilienz bei rheumatoider Arthritis.

Material und Methoden

Es erfolgt eine publikationsbasierte, narrative Darstellung des Begriffes „Achtsamkeit“ und der davon abgeleiteten psychoedukativen Trainingsmethode („mindfulness based stress reduction“) für Patienten mit rheumatoider Arthritis.

Ergebnisse

Die Ergebnisgrößen Schmerz und Lebensqualität können durch Achtsamkeitstraining ebenso wie durch klassische Verhaltenstherapie positiv beeinflusst werden. Nachhaltige Messwerte wie Funktionalität, Partizipation oder Invalidität sind von den psychoedukativen Methoden nicht beschrieben.

Schlussfolgerung

Maßnahmen zur patientengesteuerten Verbesserung der psychischen Widerstandsfähigkeit erscheinen erstrebenswert, eine rationale Analyse der Wirksamkeit wird durch Kovariablen wie Krankheitsdauer, pharmakologische Therapie und individuelle Patientenfaktoren erschwert.
