Die venöse Thromboembolie (VTE) umfasst zwei eng miteinander assoziierte Krankheitsbilder, welche auch häufig gemeinsam auftreten, ohne dass beide in jedem Fall zu klinischen Symptomen führen müssen. Unter der tiefen Venenthrombose (TVT) versteht man die Verlegung einer tiefen Vene durch einen Thrombus, wobei in gut 90 % der Fälle die Venen der unteren Extremität inklusive der Beckenvenen betroffen sind. Dabei ist es für die Therapie wichtig, proximale Venenthrombosen, welche die Vena poplitea oder darüber liegende Venen betreffen, von den distalen, auf den Unterschenkel beschränkten Thrombosen zu unterscheiden. Die restlichen Fälle betreffen in erster Linie Armvenenthrombosen, welche prinzipiell analog behandelt werden [1]. Auf seltene Lokalisationen (viszerale Venen, Sinusvenen) wird im Rahmen dieser Übersicht nicht eingegangen. Abzugrenzen ist die TVT von der oberflächlichen Venenthrombose oder Thrombophlebitis, bei welcher nur oberflächliche Venen betroffen sind. Bei Verschleppung von Thromben in die Lungenstrombahn spricht man von der Pulmonalarterienembolie (PE), einem potentiell lebensgefährlichen Krankheitsbild. Hier ist für das Patientenmanagement eine Unterscheidung in eine massive PE mit Beeinträchtigung der Kreislaufparameter, eine submassive PE mit Zeichen der rechtsventrikulären Dysfunktion sowie in eine periphere PE mit niedrigem Risiko wichtig. …
