Die Varikozele – Wann und wie soll behandelt werden?

Zusammenfassung

Die Varikozele betrifft ca. 11–15 % aller erwachsenen Männer, die Inzidenz bei einer primären bzw. sekundären Infertilität liegt sogar bei 35 % bzw. bis zu 80 %. Aufgrund anatomischer Gegebenheiten treten 90–95 % aller Varikozelen linksseitig auf. Eine Behandlung ist lediglich bei einer klinisch relevanten Varikozele mit Auftreten von Schmerzen/Schwellung/Diskomfort, ggf. in Kombination mit einer Infertilität indiziert. Eine subklinische Varikozele sollte gemäss den Leitlinien der European Association of Urology (EAU) nicht behandelt werden. Als Behandlungsoptionen stehen verschiedene Techniken zur Verfügung, unter anderem die Sklerotherapie, die Embolisation sowie die chirurgischen Verfahren, suprainguinal, inguinal, laparoskopisch und mikrochirurgisch, wobei die aktuelle Datenlage für eine mikrochirurgische Varikozelektomie spricht.
