Suizidrisiko bei somatoformen Störungen

Grundlagen

Somatoforme Störungen umfassen eine Gruppe komplexer Störungen, die aus somatischen Symptomen besteht, für die es keine erkennbare organische Ursache oder pathogenetische Mechanismen gibt. Angesichts der Bedeutung dieser Erkrankung und der Notwendigkeit, die Diagnose einer somatoformen Störung und deren Auswirkung auf das Suizidrisiko zu erklären, haben wir wissenschaftliche Literatur miteinbezogen, um die Korrelation zwischen den beiden Faktoren zu untersuchen.

Methodik

Wir führten eine Literatursuche über Medline, Embase, PsycINFO, Scopus, SciELO, ORCID, Google Scholar, DOAJ durch und verwendeten dabei die folgenden englische Begriffe: somatoform, somatization disorder, pain disorder AND psychological factor, suicide, parasuicide, suicidality.

Ergebnisse

In all den Studien, die wir in unserer Revision erwähnt haben, zeigte sich, dass das Suizidrisiko erhöht ist. Aber beim Großteil sind die Daten relativ wenig verlässlich, wenn man die nosographische Kategorie „Angststörungen, Anpassungsstörungen und somatoforme Störungen“ berücksichtigt. Zu umfassend, zu breit gefächert, um klinische Charakteristiken der Risikopatienten mit ausschließlich somatoformer Störung zu erkennen.

Schlussfolgerungen

Mehrere Studien kommen zum Schluss, dass psychiatrische Komorbidität das Suizidrisiko erhöht: Patienten mit zwei oder mehr psychiatrischen Erkrankungen begehen eher einen Selbstmordversuch. Vor allem bei einer Diagnose der Achse I verdoppelt sich das Risiko. Die Somatisierungsstörung scheint eine signifikante psychiatrische Komorbidität zu haben, insbesondere mit dem Spektrum affektiver Störungen und Angststörungen.
