Muskuläre Polyarteriitis nodosa – eine fallbasierte Übersichtsarbeit

Hintergrund

Myalgien sind ein häufiges, aber unspezifisches Symptom, das sowohl orthopädische und neurologische als auch internistische Ursachen haben kann und den Arzt nicht selten vor eine differenzialdiagnostische Herausforderung stellt. Die muskuläre Polyarteriitis nodosa (PAN) ist eine seltene Differenzialdiagnose bei Myalgien mit erhöhten serologischen Entzündungszeichen.

Ziel der Arbeit

Anhand von 3 klinischen Fällen sollen die wesentlichen klinischen und diagnostischen Aspekte der muskulären Polyarteriitis nodosa veranschaulicht und in den Kontext der Fachliteratur eingeordnet werden.

Ergebnisse

Die muskuläre PAN präsentiert sich typischerweise mit immobilisierenden Myalgien der unteren Extremität und serologischen Entzündungszeichen, jedoch häufig mit normwertiger Kreatinkinase (CK). Die entscheidenden Befunde im diagnostischen Prozess liefern dabei die Kontrastmittel-Magnetresonanztomographie der betroffenen Muskulatur, welche häufig das Bild einer Myositis imitieren kann, sowie die Muskelbiopsie zur histologischen Sicherung der Diagnose einer Vaskulitis.

Diskussion

Vor dem Hintergrund unserer Erfahrungen und der diskutierten Fachliteratur sollte bei Myalgien mit serologischen Entzündungszeichen auch bei normwertiger CK und Fehlen weiterer vaskulitistypischer Symptome an das Vorliegen einer muskulären PAN gedacht werden.
