Zusammenfassung
Durch den Einsatz automatischer Infusionspumpen in der Intensivmedizin sind akzidentelle Luftembolien größeren Ausmaßes nicht mit Sicherheit auszuschließen. Auch repetitive Injektionen kleinerer Luftmengen durch Zusatzmedikation in das Infusionssystem können durch ihre kumulative Wirkung einen negativen Effekt auf die pulmonale Endstrombahn ausüben. Die Verwendung von 0,22 μm-Filtern mit Luftabscheidung kann diese Komplikation der Infusionstherapie verhindern.
Wir haben unter Simulation von klinisch relevanten Bedingungen den Mikrofilter Ultipor Typ FAE-020 (Fa. Pall Biomedizin GmbH) in Bezug auf die Wirksamkeit der Luftelimination überprüft. Bei 50 geprüften Filtern mit insgesamt 650 Einzelinjektionen und 10 Dauerinfusionen von Luft konnte mittels Ultraschall-Dopplermethode ein sicheres Zurückhalten der injizierten Luft durch diesen Filter nachgewiesen werden.
Summary
The use of automatic pumps in intensive care medicine does not preclude the risk of accidental air embolism. The repeated introduction of small air bubbles into the infusion system in the course of additional medication may have a cumulative effect resulting in the blocking of the pulmonary capillary system. The use of 0.22 μm filters for eliminating air prevents this complication of infusion therapy. The efficacy of the microfilter Ultipor type FAE-020 (marketed by Pall Biomedizin GmbH) in preventing the introduction of air was tested in 50 specimens with 650 single injections and 10 slow drip infusions by means of the ultrasound Doppler technique. The filters reliably stopped the introduction of air into the circulation.
