Interferon-α-assoziierte Retinopathie bei Patienten mit chronischer Hepatitis C

Zusammenfassung

Hintergrund und Fragestellung: Interferone sind eine komplexe Gruppe von Glykoproteinen mit antiviraler, antiproliferativer und immunmodulativer Aktivität. Sie werden z. B. in der Therapie verschiedener Tumoren und bei chronischer Hepatitis C eingesetzt. Seit dem ersten Bericht über eine interferoninduzierte Retinopathie 1990 wird zunehmend über ernst zu nehmende Sehstörungen bzw. Funktionsstörungen der Retina berichtet, was uns zu einer zusammenfassenden Darstellung veranlasste. Patient/Methodik: Wir berichten über eine 61-jährige Patientin, die aufgrund einer Interferontherapie bei chronischer Hepatitis C eine beidseitige Retinopathie entwickelte, und geben eine Übersicht über Interferon-α-assoziierte Nebenwirkungen am Auge und das sich daraus ergebende weitere Monitoring der Patientin sowie mögliche therapeutische Optionen. Schlussfolgerung: Patienten sollten vor, während und nach einer hoch dosierten Interferontherapie augenärztlich untersucht werden, damit eventuell eingetretene Folgeerscheinungen, wie z. B. retinale Neovaskularisationen, frühzeitig therapiert und dauerhafte okuläre Schäden verhindert werden können.

Abstract

Background: Interferons are a complex group of glycoproteins with antiviral, antiproliferative and immummodulatory activity. They are used to treat different types of human neoplasms and chronic hepatitis C. Several case reports have described interferon-induced ocular side effects since the first case in 1990. Patient and Methods: We report about a 61-year-old female undergoing interferon 2α treatment who developed bilateral retinopathy during systemic therapy and provide a summary about ocular side effects and their consequences. Conclusions: Patients should be monitored by ophthalmologists before, during and after interferon therapy in order to avoid side effects like retinal neovascularisation and to prevent potential serious side effects.
