Erfolgreiche Behandlung einer therapierefraktären Akanthamöben-Keratitis mit systemischem Miltefosin und topischem Voriconazol
Successful Treatment of Therapy-refractory Acanthamoeba Keratitis with Systemic Miltefosine and Topical VoriconazoleHintergrund
Akanthamöben sind ubiquitär auftretende Organismen, die gelegentlich auf Kontaktlinsen und in deren Behältern gefunden werden. Durch Akanthamöben ausgelöste Keratitiden können potenziell zur Erblindung führen und treten in Europa mit einer jährlichen Inzidenz von 0,2 – 4,5 pro 1 Mio. Personen auf [1]. Schwere Fälle können durch eine intraokulare Ausbreitung bis zur Enukleation führen [2]. Die Therapie muss sowohl gegen die aktiven Trophozoiten als auch die stressresistenten Zysten wirken. Erstere sprechen auf verschiedene Antibiotika, Antimykotika sowie antineoplastische und Antiprotozoenmittel an. Gegen die resistenten Zysten werden über einen längeren Zeitraum topisch Antiseptika, wie Propamidin-Isethionat (Brolene) und Polyhexanid, eingesetzt [3]. Es existieren verschiedene Therapieansätze bei unzureichendem Ansprechen auf die Therapie. Einer davon ist Miltefosin (Impavido), ein eigentlich für die Therapie der Leishmaniose eingesetzter Wirkstoff, der auch gegen Trophozoiten und Zysten der Akanthamöben wirkt [4], [5], [6]. Ein weiterer Ansatz ist die Therapie mit dem antimykotischen Wirkstoff Voriconazol [7].
