Prävention der metabolischen und kardiovaskulären Langzeitfolgen bei polyzystischem Ovarsyndrom (PCOS)

Zusammenfassung

Man sollte sich bewusst sein, dass Frauen mit PCOS eine höhere Prävalenz für Übergewicht und Adipositas haben, was wiederum einen negativen Effekt auf den psychischen und physischen Gesundheitszustand hat. Eine Anpassung des Lifestyles mit Ziel „Normalgewicht“ sollte schon bei jungen PCOS-Frauen erfolgen. Ob die Inzidenz kardiovaskulärer Erkrankungen bei PCOS-Frauen erhöht ist, ist aktuell nicht gesichert, aber die Prävalenz von kardiovaskulären Risikofaktoren ist erhöht – dies erfordert ein jährliches Screening dieser Risikofaktoren. Es ist bekannt, dass bei PCOS-Frauen unabhängig vom Alter die Prävalenz einer gestörten Glukoseintoleranz und eines Diabetes mellitus Typ II (DM II) signifikant erhöht ist. Übergewicht oder Adipositas sowie eine familiäre Belastung erhöhen das Risiko, einen DM II zu entwickeln, zusätzlich signifikant. Daher sollte bei allen PCOS-Frauen ein oraler Glukosetoleranztest (oGTT) oder zumindest die Bestimmung des HOMA-Index durchgeführt werden. Abhängig von der Insulinresistenz bzw. der Glukoseintoleranz sowie der Risikofaktoren für die Entwicklung eines DM II sollte alle 1–3 Jahre der oGTT wiederholt werden. Metformin wird, nebst Anpassung der Ernährung und Steigerung der körperlichen Aktivität, als Therapie empfohlen bei PCOS-Frauen mit BMI ≥25 kg/m2 zur Gewichtsabnahme sowie verbessertem metabolischem Outcome. Inositol (in irgendeiner Form) ist aktuell eine (noch) nicht evidenzbasierte Therapie beim PCOS. Bei übergewichtigen oder adipösen PCOS-Frauen sollte unabhängig vom Alter eine Überprüfung des Lipidstatus durchgeführt werden – eine Statintherapie sollte aber nur nach sorgfältiger Abschätzung der kardiovaskulären und metabolischen Risikosituation initiiert werden.
