Prävalenz und Dimensionen von Refraktionsfehlern bei österreichischen Gymnasiasten

Hintergrund

Die Ätiologie von Refraktionsfehlern bei Jugendlichen ist im Detail umstritten. Vermehrte Naharbeit (Lesen, Einsatz elektronischer Medien) wird als Ursache v. a. für das häufige Auftreten von Myopia simplex bei gebildeten Bevölkerungsschichten diskutiert. Eliteschüler sollten daher auch häufiger und stärker fehlsichtig sein.

Material und Methode

Mit einem einfachen Fragebogen haben wir in einem österreichischen Elitegymnasium die Häufigkeit und Dimensionen von Refraktionsfehlern bei 728 Schülern verschiedener Altersgruppen erhoben. Ophthalmologische Messmethoden kamen nicht zum Einsatz.

Resultate

Insgesamt 40,8 % der Schüler der Stichprobe waren entweder myop (235 Fälle; 32,2 %) oder hyperop (65 Fälle, 8,9 %). Die relative Häufigkeit von Ametropien unterschied sich zwischen den Geschlechtern nicht signifikant. Auch in den Anteilen der „leicht“ (<1,5 dpt. 35,4 %), „mittel“ (41,3 %) oder „schwerer“ (≥ 3 dpt., 23,3 %) fehlsichtigen Schüler gab es keine signifikanten Geschlechtsunterschiede. Der mittlere Myopiegrad aller Altersstufen betrug bei 180 Schülern −1,97 ± 1,23 Dioptrien (D), der mittlere Grad der Hyperopie betrug +1,58 ± 1,37 D (n = 24). Hyperopie trat gehäuft bei jungen (10–11 Jahre: ca. 15 %) und älteren (17–19 Jahre: 11–20 %), v. a. männlichen Probanden auf, war in den Zwischenjahren aber eher selten (ca. 5 %). Der Prozentsatz myoper Schüler nahm hingegen vom 10. bis zum 18. Lebensjahr bei beiden Geschlechtern signifikant zu. Während aber bei Mädchen keine Zunahme der mittleren Dioptrienzahl mit dem Alter festzustellen war, war diese bei Jungen von im Mittel etwa −1,25 D bei 10–11-Jährigen auf fast 2,5 D bei 16–17- und über −3 D bei über 17-jährigen männlichen Probanden auffällig und signifikant.

Schlussfolgerungen

Die überdurchschnittliche Häufigkeit von Fehlsichtigkeit in unserer Stcihprobe und die erhebliche Zunahme der Stärke der Myopie im Laufe der Schulkarriere, weisen darauf hin, dass milieu- und ausbildungsbedingten Belastungen bei Gymnasiasten eine wichtige Rolle spielen.
