Perioperative Aspekte in der Behandlung Katecholamin-produzierender Tumoren

Zusammenfassung

Die Behandlung von Katecholamin-produzierenden Tumoren (Phäochromozytom, Paragangliom [PPGL]) ist eine Herausforderung für das gesamte perioperative Betreuungsteam bestehend aus Endokrinologie, Anästhesie und Chirurgie. Hämodynamische und metabolische Extremereignisse sind häufig und können zu kardialen und zerebrovaskulären Komplikationen führen. Die Morbidität und Mortalität in der chirurgischen Therapie ist aber durch moderne Behandlungskonzepte gering geworden. Die bisher als Dogma angesehene Praxis der präoperativen Alpha-Adrenorezeptor-Blockade kann intraoperative hämodynamische Entgleisungen nicht verhindern, kann aber bei vielen PatientInnen zu unangenehmen Nebenwirkungen führen und ist daher umstritten. Eine Beta-Blockade sollte nur in besonderen Situationen zum Einsatz kommen. Störungen des Glukose- und Laktatstoffwechsels sind bei PatientInnen mit PPGL häufig, normalisieren sich meist aber postoperativ. Die perioperative Therapie von PatientInnen mit PPGL sollte individualisiert anhand präoperativer Faktoren abgestimmt werden und in spezialisierten Zentren mit hoher Fallzahl erfolgen.
